Preschool Registration

Preschool Registration for the 2021-2022 school year will open Monday, March 8th 2021. Registration will only be available on-line.

Registration is processed on a first come first served basis. All required forms & documents, including birth certificate & proof of residency, must be completed in Final Forms and will be time stamped electronically.  Anything requiring a doctor’s signature – physical form, immunization records, & dental form - are not required at the time of registration.  

Students will be placed into open spots based on the order in which registration is completed. You will be notified by the Butler County preschool registrar once forms are processed.

The preschool program is tuition based and contracted through Butler County Educational Service Center.  All preschool classes will be held at the new SLSD Early Learning Center, located at 10800 Campbell Rd. (the current Whitewater Valley Elementary location).  Tuition is $1,500 for the 21-22 school year & will be billed quarterly. No payment is due at the time of registration.

Please do not attempt to input student information into Final Forms prior to March 8th. The new forms go live on this date & any information previously entered will be deleted.

Documents Checklist (Green Links are documents that you would click, print, and upload.)

Required Documents- the following must be uploaded in the Final Forms system beginning March 8 (not prior to March 8) before enrollment will be time stamped. 

  • Child's Birth Certificate
  • Parent/Guardian Driver's License or State Issued ID Card 
  • Proof of Custody/Guardianship - (if applicable) (Divorce Decree, dated stamped and signed by judge/magistrate; shared parenting agreement; custody/guardianship papers, foster placement, safety plans, protective orders, etc.)
  • Proof of Residency - Provide ONE of the following. (Additional documents may be required in certain situations.)
    • Mortgage Statement or Deed
    • Lease Agreement (must have valid dates and signed by both parties)
    • Utility Bill – Current Electric/Water Bill (NO Phone or Cable Bills accepted)
    • Property Tax Statement
    • Notarized Southwest Local School District Affidavit of Residency along with Water/Electric bill in the same person’s name that completed the Affidavit. This is to be used when parent/legal guardian and child are living in a domicile belonging to another person or leased to another person and have no other accepted residency document in the parent/legal guardian name.

Only legal residents of the district may enroll. Misrepresentation may result in prosecution.
