Vaccine Clinic Being Offered for Students

The Southwest Local School District is pleased to partner with Hamilton County Public Health to offer an on-site vaccine clinic for students age 16 and older whose parents would like for them to receive the Pfizer COVID-19 vaccine.  Dose 1 will take place on Thursday, April 29, with dose 2 following on Thursday, May 20, both at Harrison High School. 

Regardless of your decision, please log into Final Forms and complete the Consent/Attestation form titled “Consent to Administer COVID-19 Vaccine”, indicating whether or not you wish for your child to receive the Pfizer COVID-19 vaccine.  The deadline to complete this form is this Thursday, April 22. 

When you have completed the forms in Final Forms, if you choose to have your child vaccinated, you will be directed to a link to sign up for an appointment time to receive the vaccine.  The same time will be used for both doses.  Vaccines will be administered by HCPH staff on the campus of Harrison High School.

To be clear, SLSD is not requiring that students receive the vaccine.  This is obviously a decision to be made by your family, and any questions should be directed to your physician.  We are merely facilitating the process for those who wish for their child to be vaccinated.
