Permission to Administer Medication
As we prepare to start the 2022-2023 school year, I wanted to invite you to contact our school nurse, Mrs. Tessa Freel, RN, with any medical concerns that your student may have. She may be reached at tessa.freel@southwestschools.org.
For your convenience, click HERE for the form that is required for us to be able to administer medication at school. This form, as well as the forms granting permission for students to self-administer an epinephrine autoinjector and an inhaler are available HERE. Please note that any medication left over from the end of the 2021-2022 school year is no longer available, a new form to administer medicine is required each year.
